Providing adults who need daytime assistance with an active, enriching, educational and social environment

Adult and Elder Services include:

• Social interaction is increased through daily group activities and through the formation of new relationships with other clients, staff and volunteers.
• Our professional licensed staff assures a caring, structured environment that promotes mental stimulation and independence.
• Physical functioning is improved through daily mobility groups and a rehabilitative program.
• Daytime physical needs are addressed through a variety of means, including administration of medications and personal care.
• Nutritional hot lunches and snacks are served daily.
• Activities including art, horticulture and pet therapies, intergenerational programs and musical entertainment.
• The Innovative Services NW Adult Day Center promotes a strength-based and client-centered program in a nurturing, inclusive environment.

The Facility - located in a comfortable, quiet, and relaxing facility just north of Vancouver in the Hazel Dell neighborhood (7409 NE Hazel Dell Avenue). The building design provides easy access for seniors and individuals with mobility problems. A relaxing outdoor area is available immediately outside the main activity room. Outdoor plantings are maintained by clients.

The Staff - professional, licensed staff assures a caring structured environment that promotes social interaction and independence. The staff includes:

• Program Manager
• Registered Nurse
• Social Worker
• Certified Nursing Assistants
• Activity Coordinator
• Rehabilitative Aide
• Food and Beverage Coordinator
• Skilled Volunteers

Skilled Nursing and Rehabilitative Services are provided based on need and eligibility.

Hours of Services - Monday through Friday. Structured, individually-appropriate activities are provided from 10:00 a.m. until 2:00 p.m. Extended care is available from 8:00 a.m. until 4:30 p.m.
