Adult Day Health Center
Innovative Services NW Adult Day Health Center provides a structured day program that promotes socialization and delivers skilled services to support senior citizens and disabled adults. Two levels of care are provided at the center:
Adult Day Care (ADC) Includes:
- Two snacks and a hot lunch;
- Activities including art, horticulture and pet therapies, intergenerational programs and musical entertainment;
- Assistance with personal care that can include showers;
- Monitoring of weight and vitals quarterly;
- Assistance with transportation arrangements;
- Emergency plans and behavioral monitoring;
- A plan of care including oversight from the registered nurse (Physicians' orders and ongoing medication lists are obtained for quality medical oversight in case of an emergency.)
Adult Day Health Care (ADHC) includes services listed above and:
- Skilled nursing - medication administration, monitoring of unstable medical conditions, diabetic foot care, caregiver and participant education and support. The registered nurse provides skilled nursing for individuals that require monitoring of their challenging conditions.
- Restorative services - Assessment by a physical therapist and a plan of care that includes ongoing goals and objectives to improve or maintain a functional mobility. Direct ongoing services are provided by the restorative aide who keeps documentation on progress. Consultation around equipment maintenance and use is provided (walkers, wheelchairs).
Intergenerational Programs:
The Innovative Services NW Adult Day Health Center shares the first floor of the Mary Family Center with Innovative Services NW's Childcare Center. This proximity allows opportunities for intergenerational interaction and sharing of activities when appropriate.
The Staff:
Professional, licensed staff assures a caring structured environment that promotes social interaction and independence. The staff includes:
- Program Manager
- Registered Nurse
- Social Worker
- Certified Nursing Assistants
- Activity Coordinator
- Restorative Aide
- Food and Beverage Coordinator
- Skilled Volunteers
Skilled Nursing and Restorative Services are provided based on need and eligibility.
Hours of Services:
Monday through Friday. Structured, individually-appropriate activities are provided from 10:00 AM until 2:00 PM. Extended care is available from 7:00 AM until 6:00 PM.
